MOST IMPORTANT TERMS AND CONDITIONS1. MEMBERSHIP AGREEMENTThe individual signing this
agreement has paid the designated membership fees for BOX N BURN Gym and
agrees to comply with the following terms and conditions. Membership is
non-refundable and non-transferable except as specified below.

2. MEMBERSHIP TRANSFER POLICY<ul>
 <li class="MsoNormal" style="line-height:normal">Membership can be
 transferred only once to a non-member who has never been a member
 of the same branch.</li>
 <li class="MsoNormal" style="line-height:normal">Transfer requests must
 be made in writing and will be subject to management approval.</li>
</ul>

3. MEMBERSHIP RENEWAL & UPGRADES<ul>
 <li class="MsoNormal" style="line-height:normal">Renewal benefits apply
 only after completing a one-year membership.</li>
 <li class="MsoNormal" style="line-height:normal">Members can choose to
 upgrade their membership plan anytime, subject to fee adjustments.</li>

4. CANCELLATION & REFUND POLICY<ul>
 <li class="MsoNormal" style="line-height:normal">Membership fees are
 non-refundable
 except in cases of:</li>
 <ul>
 <li class="MsoNormal" style="line-height:normal">Permanent disability
 (6 months or more) with
 medical certification.</li>
 <li class="MsoNormal" style="line-height:normal">Relocation to a
 different city
 (proof of relocation required).</li>
 </ul>
 <li class="MsoNormal" style="line-height:normal">No refunds will be
 granted for temporary disabilities, vacations, or change of preference.</li>

5. MEMBER RESPONSIBILITIES<ul>
 <li class="MsoNormal" style="line-height:normal">Members must wear
 appropriate workout attire, including a t-shirt covering the chest and
 back.</li>
 <li class="MsoNormal" style="line-height:normal">Use of a personal towel
 is mandatory to maintain hygiene and wipe down equipment after use.</li>
 <li class="MsoNormal" style="line-height:normal">Members must maintain a
 respectful attitude toward staff and fellow gym-goers. Any violation may
 result in membership termination.</li>

6. GYM FACILITY USAGE<ul>
 <li class="MsoNormal" style="line-height:normal">The gym is not
 responsible for any loss, theft, or damage to personal belongings.</li>
 <li class="MsoNormal" style="line-height:normal">Members must follow all
 safety and equipment usage instructions provided by gym staff.</li>
 <li class="MsoNormal" style="line-height:normal">Lockers are available
 for temporary use during workouts and must be emptied before leaving the
 premises.</li>
</ul>

7. WAIVER AND RELEASE OF LIABILITYBy signing this agreement,
the member acknowledges and agrees that:<ul>
 <li class="MsoNormal" style="line-height:normal">Participation in gym
 activities involves inherent risks of injury, illness, or other
 physical harm.</li>
 <li class="MsoNormal" style="line-height:normal">BOX N BURN Gym and its
 staff are not liable for
 injuries, accidents, or damages resulting from voluntary participation in
 gym activities.</li>
 <li class="MsoNormal" style="line-height:normal">The gym is not
 responsible for injuries due to improper use of equipment, failure to
 follow safety guidelines, or pre-existing medical conditions.</li>

8. CODE OF CONDUCT<ul>
 <li class="MsoNormal" style="line-height:normal">Any form of harassment,
 abuse, or misconduct toward staff or members will lead to immediate
 termination of membership.</li>
 <li class="MsoNormal" style="line-height:normal">Use of
 performance-enhancing drugs, alcohol, or smoking on gym premises is
 strictly prohibited.</li>
 <li class="MsoNormal" style="line-height:normal">Management reserves the
 right to deny access to any member violating these rules.</li>

9. GENERAL TERMS<ul>
 <li class="MsoNormal" style="line-height:normal">Gym operating hours may
 be adjusted for maintenance, holidays, or unforeseen circumstances.</li>
 <li class="MsoNormal" style="line-height:normal">Management reserves the
 right to modify these terms and conditions at any time with prior notice.</li>
 <li class="MsoNormal" style="line-height:normal">Members are encouraged
 to consult a physician before starting any workout program.</li>
</ul>
